Alpine Homecare is now offering Health Insurance to all eligible employees for the rest of 2017! Alpine's health plan for employees is being provided by Humana and coverage is available for Alpine employees, their spouses, children and their families. Alpine will contribute 50% of the individual cost of the plan (unless the premium exceeds 9.5% of an employee's income, in which case Alpine will contribute the difference), all other costs will be at the employee's expense. Eligible employees include all employees that work more than 29.5 hours/week over a pre-determined period of time.
Eligible employees wishing to take advantage of this offer, need to enroll by June 30th, 2017 for coverage for the remainder of 2017 (starting July 1, 2017). Please call Alpine at 303 309 6202 to sign up or for any questions.

Please note that all eligible employees who decide to decline health insurance coverage will still need to fill out & return to Alpine the above Plan Election Form and pp. 1 & 2 of the Humana Application form.
